- Documents which confirm that it is the legal representative of the person such as a driver's license or passport.
- Documents which prove qualification of legal representative such as family register copy (created within 30 days prior to request for disclosure).
- Copy of resident's card of legal representative (created within 30 days before request for disclosure, etc.).
<Confirmation Data for Agent/Attorney>
- A copy of documents that proves agency, a driver's license of the agent, a photo ID such as a passport, and a representative of the agent that can confirm (the name and address of the agent requesting disclosure etc.)
- Copy of resident's card of voluntary agent (created within 30 days prior to request of disclosure)
- If the attorney is a lawyer, the documents for which the registration number is known.
- Power of attorney to indicate a proxy.
- About Procedure for Request for Disclosure
We will send a "Personal Information Disclosure Request" to the person by mail after identity verification. We will promptly respond to the disclosure without delay if you return the "Personal Information Disclosure Bill" in person.
As for the answer method, we will mail the answer by telephone or the answer to the contents of the request in writing.
We do not collect any fees at the request of the disclosure etc.
